Notice:
Students
in grades 6-8, who participate in school sponsored sports activities, are required to have a yearly physical exam prior to
the start of the Sports Season (basketball, softball).
1.
Part A - Health History Questionnaire to be completed by parent/guardian.
2. Part B - Annual Athletic Pre-Particiation Physical Evaluation Form – to be
completed by a Physician.
